In the Narrator application you can find the HELPINDEX file by following these steps:
- Open the Finder window and locate the Narrator application (most likely found under Applications).
- Right-click the Narrator application
- Select Show Package Contents → Contents → Resources → English.lproj (or French.lproj or German.lproj) → Narrator Help.
- You should see the HELPINDEX file in this folder.
