A TY+ file contains a TV show or movie recorded by a TiVo DVR device, such as TiVo Series 1, 2, and 2.5. It stores "chunks" of MPEG-2-compressed video and audio data along with metadata about the content, such as the TV show or movie title, its description, and recorded date. TY+ files may also include copyright protection if copyright is enabled on the device when recording the video.

You will most likely only encounter a TY+ file if you own an older TiVo DVR device and are transferring the recorded content to archive or play on a computer. When transferring the recorded content with the MFS_FTP server program included with the TiVo device, the program combines the various chunks of data into one large binary TY+ file.
If the TiVo recording does not include metadata describing the TV show or movie, the MFS_FTP program saves the video as a .TY TiVo recording file. The TY+ format is the same as the format used in TY files, except it stores metadata at the end of the file, which is what the "+" refers to in the .ty+ file extension.

TY+ files are rare since they are no longer used by TiVo DVR devices to store recordings. They are primarily created by TiVo devices released in the 2000s and only if the recording includes metadata.
Also, most TiVo devices encrypt video recordings for copyright purposes. If encryption on the device is not disabled when recording the video, the TY+ file cannot be played or converted by a computer program. The file must be transferred back to the TiVo device in order to be played.
However, if you have an unencrypted TY+ file you can convert it to a more common video format with FFmpeg. Then, you can open the converted file with a media player, such as Microsoft Windows Media Player (Windows) (Windows) or VLC media player (multiplatform).
